OptiVantage RX, LLC – Legal & Medical Disclaimer

1. No Medical Advice or Care
OptiVantage RX, LLC (“OptiVantage RX”) is a technology-enabled care-coordination and administrative services platform. OptiVantage RX is not a medical practice, a licensed healthcare provider, a pharmacy, or an insurance company. The contents of this website, including text, graphics, images, and marketing materials, are for informational purposes only and do not constitute professional medical advice, diagnosis, treatment, or recommendations.

2. Independent Telehealth Services
All clinical consultations, medical evaluations, patient intakes, diagnoses, and prescription decisions are performed exclusively and independently by licensed, certified healthcare professionals utilizing the Impact Health Network. OptiVantage RX does not employ, control, influence, or supervise the medical decisions or treatment protocols of the physicians within the network. Your utilization of telehealth services through this platform establishes a direct patient-provider relationship solely between you and the independent clinician.

3. Pharmacy and Compounding Services
Any prescription medications prescribed by an independent clinician—including, but not limited to, compounded GLP-1 therapies, Hormone Replacement Therapy (HRT), and peptide treatments—are fulfilled exclusively by independent, fully licensed, and legitimate 503A compounding pharmacies authorized to perform interstate commerce. OptiVantage RX does not manufacture, compound, dispense, label, or distribute prescription drugs.

4. Compounded Medication Disclosure
Compounded medications are customized formulations prepared by licensed pharmacies in accordance with a patient-specific prescription. Patients are explicitly advised that compounded medications are not individually approved by the U.S. Food and Drug Administration (FDA) for safety or efficacy, as the FDA does not evaluate the customized manufacturing process of 503A compounding facilities.

5. Subscription and Membership Scope
Payment of the monthly OptiVantage RX subscription membership fee guarantees access to our administrative care-coordination platform, user portal, and member-only discounted wholesale pricing structures. Payment of the membership fee does not guarantee that an independent physician will issue you a prescription. Prescription eligibility is determined solely at the clinical discretion of the licensed provider during your medical evaluation. If you are deemed clinically ineligible for treatment, your membership terms remain governed by our standard cancellation and refund policies.

6. Emergency Medical Conditions
Do not use this website, platform, or the OptiVantage RX network for urgent or emergency medical needs. If you are experiencing a medical emergency, you must immediately call 911 or proceed to the nearest emergency room.

Failed Payments and Late Accounts: If a recurring monthly membership fee or authorized medication refill charge fails to process successfully, our billing engine (Chargebee) will automatically initiate an administrative dunning retry sequence. We reserve the right to re-attempt the charge up to four (4) times over a 21-day window. During any period where an account balance remains unpaid or past due, patient portal access, active telehealth consultations, and medication fulfillment pipelines will be temporarily suspended until a valid payment method is updated and settled.

Cancellation

Cancellation Policy: You may cancel your OptiVantage RX subscription at any time, after the initial 90-Day minimum membership term. To avoid being billed for your next consecutive monthly cycle, cancellation requests must be submitted at least five (5) business days prior to your next scheduled recurring billing date. You can cancel your membership by logging into your secure patient portal and navigating to Account Settings, or by sending a written request to [Insert Support Email]. Upon cancellation, your access to member-only discounted pharmacy rates and telehealth coordination will terminate at the end of your current paid billing period.
